Hospice of West Alabama (HOWA) provides services wherever the patient is living or receiving care. It may be in a hospital, nursing home, assisted living facility, or other residential facility.
We work with patients, families, and their care providers to assure a seamless continuum of quality end-of-life care. Working closely with the patient, family, and staff, the HOWA team offers comprehensive support that addresses the physical, emotional, and spiritual concerns that can arise when a person is at the end of life.
When we provide hospice care in a facility, we work with the facility’s staff to establish a collaborative plan of care. In fact, HOWA has a dedicated facility team, designed to maintain the continuum of care by the staff and patient seeing the same nurses and social worker. We consider facility staff part of the caregiving team and provide training in end-of-life care, based on the need of each facility.
